How dermal fillers work

Fillers work in two ways, and only one of them is obvious. The first is simple physics: the gel takes up space under the skin, so an area that has lost volume, a cheek or a lip, is supported again and reads as fuller. That part you can see on the day.

The second is slower. Filler can also encourage the tissue around it to lay down more collagen, the protein that holds skin, muscle and bone together and gives skin its firmness. Collagen production falls as we age, which is a large part of why skin starts to loosen and crease. That effect builds over the months after treatment rather than showing up at the appointment, and it is one reason a result can look better at a review than it did on the day.

What dermal fillers are made from

The fillers we use are based on hyaluronic acid, a substance the body already makes and one that helps keep skin plump. Because it is not permanent, the body breaks it down gradually over months. That is also why a filler result can be adjusted as your face changes rather than being locked in.

Who should not have this treatment

Most people tolerate dermal fillers well, but there are situations where we would postpone or decline. Some of them mean waiting rather than ruling the treatment out. We check all of this at the consultation, so tell us about your health and anything you are taking.

• an active skin infection in the area we would be treating
• autoimmune or bleeding disorders
• blood-thinning medication, unless your doctor has cleared it
• a known allergy or sensitivity to any of the ingredients
• surgery, filler, laser or a chemical peel in the same area within the previous two to four weeks
